I have a Sky +HD box. The hard drive is playing up and I keep getting hard drive error messages. I've reset the hard drive a couple of times and it then works for a couple of days before the problem returns. How do I get a replacement box?

Look for the DRX890 or the DRX895 (bigger HDD)

How about if I upgrade to Sky Q and would a replacement box be free?

The typical starting point for Sky is to set a high up-front fee and then reduce this if the customer subscribes to add-ons which increase the monthly cost.
The 'VIP Upgrade' to Q is typically 'free' only if Multiscreen is added: that's close to £300 by itself over an 18 month minimum term.
