If you have a Sky Stream TV subscription then there is nothing on NOW that you are missing out on as all the same content is available on both services.
Sky Stream currently offers the benefit of UHD and HDR streams with Dolby Atmos on some content, whereas NOW is limited to 1080P and 5.1 audio with the Boost subscription. This is apparently due to change soon with the introduction of the Ultra Boost subscription which will bring UHD & Atmos to the NOW app on various devices.
Sky Stream is the better option if you want a full TV service with live channel streams of all the free-to-air services and an EPG, playlist and a selection of third party apps.
If you already have Freeview or some other TV service you are happy with and just want to watch a few shows which are exclusive to Sky, then NOW is probably the better option.

A couple of reasons to get NOWTV in addition to Sky Stream is Sky Cinema is commonly discounted to £4.99 or it a can be used as a cheap way to access Sky's subcription channels on all TVs in a home😉
