Either fan failure or imminent hard drive death, I'd think. Only a new box will cure that: Sky don't 'repair' anything at customer addresses.
Remember to retain the viewing card in a doorstep swap, and check the box carefully for cables (including a potential separate power supply) before letting the courier depart.
