My father in law can no longer use the sky remote control as it's too complex for him. Is there any way to have sky q go straight to a channel, ideally BBC1 on start up?
I know it sounds crazy but seems lots of people with cognitive issues struggle to work out how to get off the Home Screen / why it's not just like the old TV.

it is just not possible.
It is a concious decision by SKY to always present the Home Screen where they can promote Shows etc

My dad has the same problem. It's a shame Sky makes a conscious decision to make life difficult for people, especially those with accessibility issues.

It is time that sky made this optional. I am very disappointed with sky q and this is just one niggle

my mother has dementia and just got flipper remote - works brilliantly for both tv and sky box - apart from this stupid default to home page. So she now still has to remember to hit the back button on another remote. Come on sky - we will all be old soon - can you make this optional so I can default here home page to channel 101 and she can then simply scroll up and down through channels ?

It's where Harmony remotes excelled, before Logitech pulled out of the universal remote market, because you can have the 'back' button press as part of the activity start up.

The trick is to leave the SKY Q box ON - not in standby - all the time
and set up the Flipper - as per the documentation - to only turn the TV On/Off ( i.e. into/out of standby mode ).
This means that when you turn the TV on again,
you're exactly where you were when you turned it off.
In theory, it would be better to program the Flipper to turn On/Off both the TV AND the SKY Q box together,
but when I did that, the On/Off button didn't always turn BOTH devices On/Off ( for unknown reasons ).,
so,, to make things as simple as possible for my relative with dementia, I elected not to set it up that way.
It works for us.
