We have netflix premium account which allows multiple users
Is this still possible if we move to sky Q?

Assuming you subscribe through Sky
Sky Ultimate TV includes the Netflix Basic plan (1 screen + SD)
You can upgrade to Netflix Standard (2 screens + HD) by adding the HD pack On Sky Q.
You can upgrade to Netflix Premium (4 screens + UHD/HDR) by adding the Ultra HD pack (requires the HD pack first) on Sky Q.

As above, you don't have to take your NetFlix subscription from Sky. You can pay them direct and just log into the app on the Q box. (But use your TV app first if you have one)
There may be advantages in paying Sky if you require UHD on Sky channels.

The Netflix Premium discount available with Sky Q/Glass/Stream, is arguably one of the best things about a Sky subscrition. It is in our house, and best of all, you don't have to use a Sky box/TV to use it as it works on any other device which has the Netflix app.

just spoken to wrt our Netflix account which when is viewed as a premium account, but doesn't carry the premium benefits as we are unable to add on two extra users outside the household. This is a NETFLIX issue since changing their terms and conditions.

As you say Netflix tightened their policies earlier this year and extra members of the family living away from home have to be added as an Extra Member add on.
This does not negate the 4 active screens of premium memberships, as such, which still can be used at the same address but does somewhat affect many households with Students in particular living away.
Unfortunately the Extra member add ons are not currently availble for subscribers via SKY
