You must provide a copy of the bill from BT where you have been charged and a copy of a statment showing that you have paid it.

That's not entirely correct. You have to pay whatever BT is due directly to BT: that's your contractual obligation with them and Sky will not 'settle' anything with BT or any other ISP.
Sky will, on receiving the required paperwork, credit your new Sky account with up to £100 for broadband/phone (and up to another £100 for subscription television, where appropriate) which is their contribution towards early exit charges levied by another provider.
