My ITV Channels have all disappeared. I have been Googling the problem and it appears that I may need to upgrade my Sky+ to Sky+ HD - is this correct?
If so, do I need a physical new set-top box or is it just an account upgrade?

Yes, lots of channels including ITV are now HD only
You need a HD compatible box a Sky+ box is incapable of receiving a HD signal

Yes, lots of channels including ITV are now HD only

You would need a new box, from Sky you would need to upgrade to a Sky Q box. But if you can purchase a Sky+ box then got for a 890 or 895 box as these are capable of receiving ITV and BBC HD channels.

The BBC ceased SD satellite broadcast entirely on Monday 8th, while ITV switched off the SD versions of ITV3, ITV4 and ITVBe, plus Border Scotland, Channel, UTV and Wales, on Tuesday 9th.

My mother and father had similar issue but their sky box was already HD. The solution which worked for them was to force the sky box to upgrade the software.
You can do this by powering off and then holding in the backup button when you powere it back on. This eventually forced the box to upgrade the software (which takes around 10 mins). Once the box came back on the planner showed both BBC 1 and 2 channels and ITV which had previously been hidden.

I'd note that the box may have been running the most recent version already: Sky has basically stopped releasing updates for that platform. However this process would also have rebooted the box, which may have been what resolved the issue.

An upgrade from SKy would be to SKY Q (or SKY Stream - the internet streaming version)
In which case you may wish to read some notes I have made for people considering switching from SKY +/HD to SKY Q
