Been watching 'golf's greatest holes' but episode 4 of season 2 is completely missing. Has anyone got any idea of where I can find this, and why it's missing?

That episode does seem to be missing on Series 2 on my Sky Stream also.
Your best bet to get this checked and resolved by Sky is to send an email to the Sky Viewer relations team to report it; ViewerR@sky.uk They do respond to emails with any updates for your queries or problems.
