Insurance claim info
I need to find proof of purchase and proof of usage to submit an insurance claim for a stolen phone. Does anybody know where I can find this info:
Proof Of Purchase in a PDF Format, containing the below information:
Name and address
Make and model of device
IMEI number (mobiles only)
Date of purchase
2. Proof of Usage from your Network Provider in a PDF Format, containing the below information:
Name and address
Make and model of device
IMEI number (mobiles only)
Date of first usage
Date of last usage
Date and time of blacklistinG
