Wow guys I am currently going through the EXACT same issue and I'm at the point I've had city fibre out three times and been sent back to the network team for the fifth time,
DID ANYONE FIND OUT THE FIX?
im a field engineer aswell and its been 4 weeks and no one has a clue what's happening?

@CLO+bskyb Sorry to hear that, is your setup the same as all in the post, was a recent posting seperate from this exact same, and the OP got so fed up, ordered a sky Max hub popped that on and it connected fine, and as you can see everything and i mean everything was tried to get that SR203 black sky Broadband Hub to connect but it was stubborn and flat out refused to do so lock stock and two smokking barrel's....
Ending part was @SkybrokemyBB- had vodafone trot up stick there hub/router on the connection and guess what it worked first time, and he now has all the sugar from sky to deal with! Happy to do that he is/was and after such a long time down was well p'd off at sky to say the least!

I can't begin to describe the nightmares that SKY put me and my family through. Absolutely disgusted at their so called Customer Services. They tried and tried to offload their technical ineptitude onto City Fibre, who were superb throughout. Once I eventually binned SKY and went with Vodafone it was like night and day. I simply plugged in their router and it immediately worked and super fast!! Without Jim I would have gone insane. The Ombudsman is on the verge of ruling on the amount of compensation that SLY owe me.

I believe SKY know they have an mismatch issue with their router and the CF ONT box either that or they aren't inputting the correct serial number of the ONT box. Depending which customer service person you get from SKY, one will say the serial number is unimportant and another will say it's vital??? The best advice I can give you is get shot of SLY as quickly as possible. But make sure that they issue a "Deadlock" letter first. I eventually went with Vodafone and their system worked perfectly from the word go and from the very same ONT box that SLY kept on insisting was faulty. Jim is worth his weight in gold BTW with his advice, he should be running Sly's Technical Team.
