Note switch credit is limited to £100 per service (service = tv (currently only applies to sky Q) or broadband)
the documents that need to be sent in pdf are a copy of your final bill clearly and separately identifying the termination charges and a copy of the statement showing it's been paid

You must provide a copy of the bill from BT where you have been charged and a copy of a statment showing that you have paid it and send these to Sky via this email address - switching credit @sky.uk (all one word without the gap)
