What device are you using to try and access the Apple TV+ app? A Sky Q box? A Sky Stream puck? A Sky Glass TV?
How have you signed up for the Apple TV+ trial? Have you created an AppleID and password?

Hello madam, I'm in sky glass and already have an account but when I sign in it repeatedly asks me to sign up again, I have been watching it prior to this episode

Have you tried rebooting your Glass TV? Turn it off at the wall for a minute then power it back on and try to login again.
Alternatively try refreshing the apps:
To refresh apps on your Sky Glass TV, press the Home button on the remote, go to Settings > System Management > Resets & updates, select Refresh Apps, and wait for the confirmation message
It can take hours for the refresh to fully complete so it's best to let it do it overnight. All the apps on Glass are hosted on a server so you can still turn the TV off after you've started it running.
