Issues with phone line

My elderly father is having issues with his phone. Other people can hear him but he cannot hear them. The voice light is not on (Any colour) on his router. He does have a microfilter on the phone line. His broadband is fine. I connected an old fashioned corded phone directly to the phone line (having removed the microfilter) and issue is still there. Can you help please
