I've been with sky for 22 years now but am now a little behind. Even though I have been paying for multi room all year I only have one sky box which is sky +. Its very old now so time to upgrade but to what? Sky Q and use the muti room to add a second tv orrrrrrrr sky glass but does that have multi room.

The equivalent to Multiroom in Sky Q is Q Mini boxes over WiFi with a Multiscreen subscription.
In Glass it's Stream pucks with a Whole Home subscription.
There's also Stream by itself (no Sky-branded television set)
Note that Glass/Stream is not an 'upgrade' to Sky+ : it's a very different system.
The 'upgrade' from Sky+ is Sky Q, although note Q does not have Reminders or Slow Motion.

If I was simply to remain with the single TV, and baring in mind that my sky + box is VERY old now, whats better to go for, Sky Q or change sytem to Glass?

I'd suggest Sky Q, but be very clear with Sky that you want no increase in monthly cost and no up-front installation fee, otherwise they'll sneak in a Q Mini and charge you for it.

If your Sky+ box is not an HD model, I'd suggest this route:
