If your new home doesn't already have a Sky+ compatible satellite dish and cabling installed and you want to take Sky+ with you then you'll need to arrange for an independent satellite installation company to attend. This will be at your own expense.
Sky+ is an obsolete legacy system, no longer supported by Sky. You can still receive it for as long as your Sky+ box lasts, but Sky will no longer replace or install the necessary equipment for the service to run.
Once you have a dish with appropriate LNB and cabling installed, connect it to your existing Sky+ box and you should be good to go. Let Sky know your change of address though.
