What is the make, model and age of the TV you are trying to use the puck on? Have you used the Sky supplied HDMI cable?
Sky Stream uses HDMI 2.2 standard and the TV needs to be compatible with this. Some older TVs arent compatible, some TVs may need a HDCP setting turning on in the menu etc and need to ensure using a compliant HDMI cable.

That's potentially a fault with the HDMI cable. Make sure it is fully connected to both the puck and the TV. Give the puck a reboot too (turn off at the wall for a minute then power back on) and see if that helps. Check the video output settings - if you're using a 4K TV then it should be on 2160P but try on 1080i which is the native HD resolution.
