I've just upgraded from Sky HD+ to Sky Q and the software version the box came with is Q190.000.16.00L. I see that the latest version is Q220.000.23.00L. Should I be updating (manually) or is it safer to stick with this version? I can't seem to find a version tracker which detailks the changes made between versions. Have had bad experiences with Windows updates causing more problems than they solve, so I'm a bit wary.

Yes it is safe to do a manual update - there isn't a change log for releases but I think some apps maybe part of it & I know a couple of bug fixes.
I would definitely recommend updating
