What are the signs that the hard drive in my Q box is starting to fail?

An error should display but possible symptoms are:
Recording failing
Unable to rewind live TV
Mini boxes not working
Extended time switching channels

Thanks @PandJ2020 @MightyQuinn for the info. Im just concercened as when i fast forward it doesnt seem as fluid as it used to be if that makes sense.

You could always try a hard drive reset, often helps with such issues.
Follow the Sky help instructions here;
Remember: Resetting your hard drive will delete all:
- Recordings
- Downloads
- Purchases
- Reminders and series links
