If you've fully paid it off then you cannot 'swap'.
You can purchase a new device if you wish - you can turn 'old' device into cash using many trade in services.
If you want to keep the dataplan (and phone number) then you simply move the SIM from old to new phone. (So do not activate any new dataplan)

So the old data plan will still work then, dam - just ordered new phone and data plan... the new plan seems better though, I'm sure I can keep the same number for her too?
will the old sim data plan stop or do I need to cancel it then?

If you move that sim to the new phone then yes you can. If you want that number on the new sim you will need to use a pac code to transfer it to a PAYG network then back again to the new sim. The new sim will have a new number.

You would need to call Sky on 150 to give 31 days notice to cancel it.

If you activate the new SIM then it will be an additional dataplan with a new phone number.
if you don't activate it then you won't be charged and it will drop off your account.
As noted above, there is a protracted method of porting the number out of the network and then back in if you want the better offer and keep the number.

That is correct as it will be automatically cancelled.
