Proof of usage item

Can you please providing partial proof of usage for my mobile phone. Please be advised that proof of usage is required from plan inception 11.11.2025 to the date of loss reported 10.12.2025

How I can get partial proof of usage item inception date and loss date as sky protect company ask these documents for claiming
