New phone and SIM card.
Sky ID is linked but nowhere to put the activation code. If I retry, i get a message stating the account is already linked then taken back to the homepage.

If you can't activate the sim due to this error you will have to find away to contact Sky so they can activate it for you, there are links to their social media sites at the bottom of the pageĀ if you do not have access to another phone.
