I just joined Sky last month and have been missing calls due to the lack of call waiting. How do I get this set up.

Call waiting is an optional extra so you will need to call Sky on the free 150 number and ask them to activate it for you.

What is the monthly cost, and can it be cancelled at anyime

The price is shown in the link in my post above yours and yes you can cancel it anytime you want.
