Adverts in the wrong place

If we are to be force fed ad breaks in on demand content, please at least insert them in the correct place! How hard can it be? You're watching the normal American style documentary shows which naturally lead into set segments where a traditional ad break would be. Then... nothing, the show continues for a couple of minutes then the ad comes in mid sentence, seemingly with no thought at all. Just ridiculous.

Still not fixed, months later. On demand content - the 'ad break title card' shows, the second 'ad break title card' shows, the programme recommences and then the adverts play. How hard can it be!
