Have you activated your included Disney+ through Sky firstly?
That and making sure you are trying to login with the correct details are the usual issues.
If you have, then as this is an app issue on a 3rd party device, I would reach out to Disney+ help to troubleshoot. You can start online here and then use chat or even give them a call, they are very helpful.
