Refunding an early exit fee for another supplier

We've recently transferred our broadband to Sky but were still under contract with another supplier.  Sky agreed to pay the early exit fee if we transferred to them.  Can someone tell me how we obtain this money from Sky once we've paid the early exit fee?

@SuperSec3 Once you have settled the bill with your previous provider first, This link explains how to apply for switch credit

Note switch credit is limited to a maximum of £300 (service = tv £100 and/or broadband £200)

the documents that need to be sent in pdf are a copy of your final bill clearly and separately identifying the termination charges and a copy of the statement showing it's been paid

Note you must apply within 90 days of your sky service activation date
