How do I get Sky Superfast Broadband, Entertainment, Anytime calls and mobile as one package?

That's not possible: there are too many different contracts, regulations and financial agreements involved.
Television and broadband is frequently co-sold (you'd be offered Sky Broadband part way through the online pathway for signing up to Sky Television) but there's no crossover with Sky Mobile at all.
Once you have a contract for either television or broadband, you can use the same login for the Mobile website (but having those contracts doesn't mean you'll automatically pass a Mobile credit check)
