You must provide a copy of the bill from Vodafone where you have been charged an early termination fee and a copy of a statement showing that you have paid it and send these to Sky via this email address - switching credit @sky.uk (all one word without the gap)

Note that Switch Credit is not a 'reward': it's a contribution towards applicable early exit fees charged by a competing provider and so at best balances out that particular expense by putting an equivalent amount against the Sky account.

 

If you are expecting cashback for signing up through a third party, that's typically not the responsibility of Sky to pay.
