I need proof of usage for my device

I lost my device and for its claim I need proof of usage document and I have requested this so many times but none of the sky advisor giving me proper document every time they tell me that I will get this in 7 days but every time they fail to do so I need full usage proof from day one to last day with IMEI number model etc
