What is the name and address of Sky Communication Provider?

I'm filling a form to move my number from Sky to a new provider. I have to write in the name and address of the 'Losing Communication Provider',  This means Sky, but I'm not sure if I should use that name, and I certainly don't know the address. Can anybody help?

@Ilfracombe2  If you are moving from Sky to another provider then you must check that you are not still under your minimum contract with Sky as you will incure extra charges and when you sign up for your new provider they will tell Sky that you are leaving.

@Ilfracombe2  As the switch is between broadband providers you should need to be asked for a communications addre3ss, no other customer has ever been asked for this.

You new provider should be able to process this without such details, if not look for one that can.

Well, they are asking for it, and as they are the only full fibre service in the area, I want to stay with them. This is what they write in their communication to me in instructions for completing their form:

They seem to think it is simple, but I can't find their address anywhere.

Very odd. Anyway Sky's address is in your contract, as follows:
