In Sky Q you need the Sky HD add-on (Netflix Basic to Netflix Standard, so two screens with HD output) and then the Sky UHD add-on (Netflix Standard to Netflix Premium, so four screens with UHD output). Sky now typically bundles HD and UHD together at a lower price.
Note that bundled Netflix does not have the Extra Member feature for use in a different household, and that a Q Mini box cannot output UHD from Netflix or anything else.
