I currently have sky Q and I am considering adding Netflix to my package, do I need to have a Netflix account already or when I upgrade my package do I set up a Netflix account . totally confused can't find any help at all on this

No you not need to have Netflix already if you want to take it from Sky.
Netflix on Sky is termed Ultimate TV .
Assuming you subscribe through Sky
Sky Ultimate TV includes the Netflix Basic plan (1 screen + HD)
You can upgrade to Netflix Standard (2 screens + HD) by adding the HD pack On Sky Q.
You can upgrade to Netflix Premium (4 screens + UHD/HDR) by adding the Ultra HD pack (requires the HD pack first) on Sky Q.
