You don't need a PUK code for a Glass TV or a Stream puck. That's a Personl UnlocKing code

for mobile phones. 

The delivery company should bring both your Glass TV and Sky Stream Puck, if that is what you have ordered.

In addition to the above, you will only get a Stream puck if you also ordered that and added the Whole Home pack to your subscription, if you jsut ordered the Sky Glass TV you won't be getting a Sky Stream puck as you don't need one.
