We had a Engineer from Open Reach instal our SKY on 8th September but did not ask us to sign confirmation - on same day we linked up with expert from SKY whom ran us through how to link up broadband etc with our telephones which were using information and both by our lap tops. We are now linked up I assumed Sky agent would have confirmed linked up action completed

There's typically no 'confirmation' required: it's unusual that you actually got to speak to a human, as activation is often carried out entirely remotely.
