Permission to work form

I have a full-fibre upgrade scheduled to be installed on June 4th, and I need Sky to send my housing association, Livewest, a permission to work form. Does anyone know who to contact to get this arranged?

@Lydia7 it isn't Sky who send these forms it is Openreach or City Fibre as it is their networks Sky use and who does the work but call Sky as they should be able to intiate that for you.

Housing Associations seem to delight in playing silly games with tennants: they must all know by now that FTTP installation is happening nationally, that it inevitably involves some work being done on the property (including drilling) and that Openreach really isn't great at communicating with occupants.
