@JPR007 @TimmyBGood @Chrisee @JimM1 @GD1 Thanks everyone for helps.
update: Today, an Openreach engineer carried out some work outside my flat. It turns out my setup was fine; the issue was due to broken wires located externally. I'm not sure where exactly as the engineer drove off for an hour, but after returning, the connectivity issue was resolved.

update:
my upstairs neighbor mentioned that their landline and broadband stopped working on the same day the engineer fixed my connection issue, which makes me wonder if the Openreach engineer might have fixed my connection but accidentally caused issues with theirs. They've already contacted their provider, and an engineer is due to sort it out. I'm just hoping this doesn't mess up my connection again. I’ll be keeping an eye on both our connectivity issues. 🙄

Unfortunately that's quite possible, and the distinctly non-standard nature of that faceplate does make me wonder about the nature of the building wiring.

Sky and Openreach should implement a protocol for conducting a pre-connectivity assessment to ensure service reliability before confirming activation dates with clients. 😶
It leaves me questioning the level of professionalism and competence displayed in this situation.😶

Which would likely push up costs, OR don't do such a service and Sky can't do it for them nor make them in the current process.
