How do I get sky to send me Final Bill letter

I am moving to EE and they will pay the £165 early termination charge that sky have taken however EE will only reimburse a letter that has Final bill on it from sky 

please can Sky do this it needs my name and address on it aswell and date of termination

Sky won't provide any different format to their normal bill which you can download as per the instructions provided by @Mark39 .

Its worth noting this "the bill isn't in the correct format" nonsense is just a stupid game that all ISP's provide in order to make it as difficult as possible for you to get them to reimburse the early termination charges; they all know exactly the format of the final bill and how the early termination charge is listed on each other's bills.
