Final paper Bill

So I switched from Sky to EE One as it was cheaper and got free mobile account. I've been asked by EE to provide a final bill with: 1. Name& address  of account holder

2. Debit Adjustment

3. Disconnection date of previous supplier

I can't seem to get a paper copy anywhere
