Last episode missing from "Tales of the unexpected" Series 2

Why is series 2 episode 16 of "Tales of the unexpected", "The Man at the Top" missing from schedules?

Thanks for suggesting I email viewerR@sky.uk. I emailed as suggested the same day as your reply (September 3rd), and I received 'Acknowledgement from Viewer Relations (KMM67976598V69233L0KM)', and a "Thank you for your enquiry. We will respond to you within 3 working days of receipt of your email." So far, 8 working days later, no other reply.

The following URL shows only 15 episodes available in series 2. The last episode, 16 "The man at the top" is still missing. This appears to be the only missing episode from all the series of this show.
