We currently have three Sky HD+ boxes around the house on one account with multiroom, the main one in the lounge and the other two in bedrooms. All of the boxes have full Sky packages.
We are considering replacing the main box with a Sky Q box.
Would this be possible or would all of the boxes need to be changed? It's doubtful that we could use Skystream due to our rural Internet connection.

As @GD1 says, unfortunately you can;t mix Q with SKY HD - the account is one or the other also as mentioned SKY HD boxes use a different feed from the dish to SKY Q
You may wish to read some notes I have made for people considering switching from SKY +/HD to SKY Q
BTW SKY no longer issue the Hybrid LNBs mentoned

@quislet You wouldn't be able to carry on any subscriptions on the old HD+ boxes as you can't have Sky Q & Sky+HD subs at the same time.
You would also need a hybrid LNB so you can keep using the older Sky+HD boxes as freesat boxes (Access to free channels only such as BBC, ITV, Ch4, CH5 etc).

@quislet you would need a Sky Q box to replace your main Sky+HD box and two Sky Q Mini boxes to replace the other two Sky+HD boxes.
Only the main Sky Q box would be connected to your dish. The 2 Mini boxes connect to the main box over wifi.
