I think all that was changed in .23 is that the sound is now ahead of the picture. If I adjust it back to 40ms, everything is aligned. This is at least better than having the sound already lagging, where we can't adjust it.
So, at least we can get it working acceptably, it just doesn't work out of the box for me.
(Only tested iPlayer last night, will test Sky channels later too).
Set to 'passthrough'.

@Pikman just been watching sky this morning and the lip sync is off again on passthrough, audio is delayed on mine so can't fix it. It was fine yesterday don't understand it.

Are you also on version 23?

@Pikman no I'm on 22, it's been saying updating software for the last few days though.

Mine moved to 23 last night, it does seem better!
It was on 21 beforehand and turning it off/on at the wall seemed to prompt an update 😅

Thats a known bug, your Glass/Stream device will update automatically to the latest version, which is normally released in batches.

I've found through all this that it's only really the sky/tv channels that have the unfixable lip sync problem. Most things I've streamed from services like Netflix / Prime / iPlayer etc seem to be fairly fine.
My software updating experience this time was a few days ago I got a mainly blank blue screen on startup with the words along the lines of 'please wait...' eventually it then turned in to the Home Screen and I went straight to Settings all the way at the bottom of the screen (btw wish you could just press up at the top of the Home Screen and it loops round to the bottom for quick settings access). But that time after doing that my software hadn't been updated so I don't know what the 'please wait' message meant. But then the same thing happened yesterday but this time it went from QS022.. to the latest QS023.007.00P.
I need to test more but I still noticed some lip sync problems on sky news last night. As others have said it does seem to vary from not too bad to very noticeable and therefore very annoying.

I've had Sky Stream for nearly a year and lip sync is still variable. It has been on every software version.
It depends on how sensitive you are to noticing it. My wife doesn't care one iota, but I notice it immediately.
I've started just using Freeview HD for our terrestrial viewing - we're lucky enough to still have an aerial on the roof and signal strength is excellent. It never has lip sync issues, and I can record programmes from the EPG to a USB flash drive, so can fast forward through ads with ease.
Sky Stream is now only used for occasional viewing of Sky programmes. Every time we've channel-hopped with it when watching live TV there seems to be an issue, either with lip sync or the remote will just go unresponsive. I've had replacement pucks and remotes but they've all been the same. Unreliable.
I reckon we'll just cancel it as we're using it less and less.

@Nicky_Jinx yeah we're kinda the same to be honest. I've got a virgin stream box too which is free and there's no issues. The menus and everything don't look as nice but at least the audio is right.
I don't even watch the sky stream anymore. It's too annoying. Cancelling when the contract ends.

Why does this thread have the green banner saying "answered" when as of even today more users are reporting this issue? All I see is that it has been "raised with Sky on the closed forum" which is not really an answer. We have three pucks/TVs and the change from long run, failing dish cable connections has otherwise been terrific since we got Stream in September. But only about a week ago we began having problems with voice dialogue missing and lip syncing. Strangely the dialogue works if we select Dolby/Dolby Plus but does not work via PCM, Auto or Pass Through.
For any other Samsung users - we now are stuck with the Samsung smart TV menu "searching for remote" which we can't seem to undo.

That's a shame @Nicky_Jinx isn't it, as there are good things about it but the bugs can undo all the good. I've had other issues too that I've not included in this thread to keep it on topic like rewind and fast forward experience on tv channels. Sometimes it gets stuck or doesn't show that it's rewinding etc and then sometimes during these times the puck can even crash (become unresponsive). I keep hoping these things get sorted with each software update but doesn't
seem to happen. I'd actually like to see a software update log file listing the bugs that have been squashed and new features too (bit like with Apple updates etc).
I too still have my old sky plus box that I occasionally use still. Mainly to watch Talking Pictures channel as that still isn't available on Sky Stream/Glass.

Oh don't get me started on the rewind/fast forward issue... LOL.
I've been in touch with Sky directly multiple times over the past 10 months and have told them about all these issues but it all just seems to fall on deaf ears. I've had my subscription cost discounted for the inconvenience, but like you, I'd just like it to work properly.
It has potential to be an excellent service but the speed at which it's (allegedly) improving just isn't anywhere near fast enough.

To be honest, like most here I also have problems with the audio sync on my pucks, and they are now on the Q0023 version. It drives me up the wall. Sky stream is my main tv source and I was forced into it after my Sky Dish started getting obstructed by trees. I am beginning to wonder whether I should move to another tv source after 20+ yrs with Sky.

My contract is up in June, after 14 years I'm leaving. I'll use an Apple TV and get the now tv app for sport and subscribe to Netflix separately. The lip synch issue drives me mad - I cannot believe that the team responsible do not use sky stream in their personal life (because if they did, the issue would be fixed).
