Further testing complete, the issue is better (as in less frequent), but not completely fixed sadly.
Instead of the out of sync audio and then skipping video like before, when it breaks, I get a freeze of video, then a black screen, then a "Loading..." wording on screen before the video restarts having skipped backwards 10 seconds or so.

So far the issue is consistent on my main Q box as described above - mostly just a sound drop out thing (I say "just" - it remains a big issue). One further observation is that when I paused only about 3 seconds into a video, when I came to rewind before playing it rewound to the beginning not 10s of course. Immediately I had a blank screen, "loading" and then silence so the "fix" does appear to need to rewind to an index point.
However, on the one mini I have tried it is exactly as @stereohaven describes.

If version 20250304_15_RC00 is the most up to date version, the issue is still there. It is less frequent, but still a major problem. The first couple of times I paused YouTube and unpaused it, the video continued and i thought the issue was solved. Unfortunately I was wrong. Sometimes when you unpause, there is a slight break in the sound and the video carries on. Most times, however, the picture freezes, a 'loading' message appears on top of the freeze frame, then the screen goes black with a 'loading' message and the video resumes with about 10 seconds of rewind. Getting absolutely fed up with this. I have a basic Sky package and only use Sky for its ability to record live TV and its Youtube, Netflix and Amazon Prime apps. I use Youtube the most as an app. I'm sure there are alternatives out there with a one off purchase cost?

it's slightly better but still has issues. after unpausing I lose sound for about 10 seconds then it comes back

@tmci i suggest Amazon fire stick. All three apps available and no YouTube issues

I have app version web_20250309_03_RC01.
The issue unfortunately still exists. It seems to happen slightly less but still happens often and you don't know when it will happen; so still better off pressing rewind once after you un-pause to mitigate the issue, as it wastes less time than when it freezes.
