its simple the home button on my sky Q remote changes the hdmi of my Sony tv from hdmi 1 to hdmi 4

Try completly resetting your Bluetooth remote.
Press and hold 7 & 9 together on the remote until it’s led flashes four times.
Then make sure that you are pointing your remote at the Q box and press 1 & 3 together until the led flashes and follow onscreen instructions to re- pair.
Also try rebooting your Sony TV.
Latest models will reboot if you hold the Power button on the remote for more than 3 seconds. Otherwise disconnect power lead for 30 seconds and then reconnect.

Just incase anyone else has this issue... mine was doing the same and to fix it I went to settings on my sky stream puck, then to startup and standby and switched hdmi control to off. It was fixed right away

Was it switching to an hdmi input that the Stream wasn't connected to?
