I assume you are just talking SKy Q where the additional boxes are called miniboxes (Pucks are specifically for SKY Glass & Stream)
You would need an additional mini box - there is usually an initial admin fee which I think is currently £50
Use of the mini box is covered by your existing multiscreen subscription
You may already know but just in case you don't - you can use a maximum of 2 minis at the same time for SKY content - Satellite/Recordings- (only one if you have the non UHD box - Hardware version 32Cxxx) although you can have up to 4 on the account
You may be able to use more at the same time if the first ones in use are only being used for Streaming apps such as Netflix or Disney+

Not sure what set up you really have but you can't mix platforms, so you can't have a puck (Stream) and a Sky Q box.

Q Mini boxes are loaned, not sold. Sky charges an up-front fee for each box after the first because they earn no additional monthly revenue from the Multiscreen subscription supplement for the second, third and fourth Mini.
