Wrong Phone on account details

About 1 year ago I cancelled on of the phones on my contract, an iPhone SE, and all seemed well. 
Now my account says the iPhone I have is the old SE while the phone I kept has vanished. I still have the number and it still works. 

How do I correct the phone type on my account????

Did you cancel it within the 31 day cooling off period and then returned it to Sky, if so you will need to speak to Sky about this on 150.
