It carries over and you just put your sim card into the new device.

Any new dataplan you order (regardless of whether you activate it on a SIM or eSIM) and activate will have a new phone number.

You can convert an existing SIM to eSIM but generally there's no need - simply move the SIM from old to new phone as above.
