How does it cost to upgrade a mobile phone on a contract with sky?

Nobody here can't answer that as it depends on what device you, how much left there is to pay on it. When you upgrade the device you return still has to be paid off so if the value Sky says it is worth isn't enough to pay it off you would have to pay the difference on the next available bill.

I have 12 months left on my contract and I'm not looking at leaving just wanting a new phone as mine is damaged.

A damaged phone will reduce the swap value, potentially significantly.

If you do swap then any shortfall from the amount of credit owed will be payable in full on the next bill - this could be many hundreds of pounds.

What sort of damage?
