Ex mobile phone on my contract

Ex partner has a phone on my account. I thought the contract was expiring in April next year, but there's another 12 monyhs after that!

I am a survivor of domestic abuse & just want to wash my hands of anything connecting me to him. 
Is there any way to do this please?

You can cancel the SIM but you have to keep paying for the phone.

He's had the phone since April 23, would he be able to request a PAC code & move supplier?

A pac code moves the number from the sim so yes this would be possible to do as this can be done once you have had the sim for 12 months which he clearly has.
