Try logging in to my sky (www.sky.com/MySky) and then navigate to https://www.sky.com/productsettings/streaming-tv/activate.

 

Also Sky recommend a reboot of you Glass device: https://www.sky.com/help/articles/setting-up-sky-glass-linking-unsuccessful

 

However if this still doesn't work, you will need to speak to Sky (we are fellow subscribers here).
