How to remove stuck shows on playlist

How do you remove shows from the playlist?

I've tried removing it and it still shows so I tried adding and removing, still there, even tried power off the box and back on again.

Click Playlists 

With remote scroll to bottom

Click Manage All

Then click on item you want to fully remove 

If you mean Items showing on continue watching rail on home menu screen that's different to playlist
