Broadband Speed/Broadband Guarantee

Hi, 

We have been experiencing a broadband speed below our average for the last few weeks. 
Last week we had an average of 10.2 and it has now gone down to 1.3. 
We have been made aware that anything below 12.4 for 3 consecutive days we are entitled to the Broadband Guarantee. 
Please could you advise next steps to claim this.
